MHSAAáPhysical Form - A physical is required before any student can tryout for or participate with a team.á The physical must be dated on or after April 15 to be considered valid.á A completed form is kept on file on school year, not one calendar.á The reason, no school sport begins after April 15th and therefore would not be valid. áClick on this site for the form. áhttps://www.mhsaa.com/Schools/Health-Safety-Resources​á áI suggest the 2 page rather than the 4 page format.

Parent-Coach Communication Guideá-á Communication is vital to both the parents and coach.á The Parent-Coach Communication Guide is designed to offer ideas and suggestion to parents in regards to communicating effectively with their son or daughter's coach.á It also suggests what parents can expect from the coach.

Student-Athlete Handbook - The student-athlete handbook is designed to inform you of many facets dealing with athletics.á If you can not find the answer to something you are looking for, please contact the athletic office.

MHSAA Concussion Protocol & Return to Competition Formá-áShould an athlete suffer an apparent concussion, the MHSAA Concussion Protocol will serve as the guide for the athletic trainer, coach, parent and athlete. áBefore returning to practice/competition, the athlete must have the appropriate paperwork signed by an MD or DO.

Volunteer Chaperone Paperworká -á Any adult volunteeringáwith a programá(whetheráthat be in a coaching capacity or driving a vehicle to a contest) is expected to fill out and turn in volunteer chaperone paperwork.ááA background check willábe run on the adult.á

Volunteer Driver Paperworká -á In addition to the volunteer chaperone paperwork, an adult willing to drive athletes to a contest is expected to fill out and turn in volunteer driver paperwork.á Your license number is required and a background check will be run.á Insurance information is also required.

Hall of Fame Nomination Formá -á Coaches and athletes (10 years after graduation) may be considered for induction.á The hall of fame nomination form must be submitted to the athletic director who will present the candidate at the fall committee meeting.á Candidates will be considered for three years before paperwork needs to be updated and resubmitted.

Kiwanis - FML Athlete of the Month Formá -á This is the application for the student-athlete selected to represent Holly HS.á Criteria includes both sport and academic accomplishments.

MHSAA Educational Transfer Form - This is the form used when requesting a transfer student gain eligibility due to a situtation covered in this paperwork.
